As a newsletter editor, I can see how errors slip out.
It happens to me all of the time...with every issue of the newsletter
there are a couple of errors that don't get caught until after it gets
printed.  This is in spite of the fact that I proof it twice, and a second 
person who also proofs the newsletter.  Every single time I ask myself,
"how did that one get by me?"
